On February 27, 2026, CMS announced a vigorous plan for anti-waste, anti-fraud measures in Medicare, with strong highlighting given to two areas: (1) durable medical equipment DME, and (2) genomic testing. The initiative, abbreviated CRUSH ("crush fraud") is open for public comment until Monday, March 30, 2026.

Genomics fraud includes highly improper billing of hundreds of millions of dollars for genetic testing that is impossible or pointless in a Medicare population. (These occurences were vastly dominated by the states of Florida and Texas where Medicare payment controls were amazingly weak, for years.) An example of a $52M genetic test scheme is here.
